How We Support Your Transition
Post-Discharge Psychiatric Follow-Up
Continue psychiatric care after leaving a hospital or treatment program. We review your discharge recommendations, current medications, symptoms, and recovery goals to develop an individualized outpatient treatment plan.
Medication Continuity & Bridge Care
Short-term medication management for adults establishing ongoing outpatient care after discharge. We review available records, assess your current needs, and prescribe when clinically appropriate while coordinating the next steps in your treatment.
Long-Acting Injectable Medication Support
Continue prescribed long-acting injectable treatment after discharge. We review your medication history and injection schedule, coordinate with your treatment team, and arrange administration and follow-up when clinically appropriate.
Addiction & Recovery Support
Continue outpatient treatment for substance use and coexisting mental health needs. We provide individualized medication management, monitor your progress, and, with your permission, coordinate with your recovery supports to help you stay connected to care.
Mobile Nursing & Community Visits
Receive mobile nursing and in person support in select areas, by appointment. Our team provides medication administration, education, monitoring, and follow-up in homes and community settings, based on clinical needs and service availability.
Care Coordination & Referral Support
With your permission, we work with discharge teams, therapists, primary care providers, and community programs to support your transition. We review treatment recommendations, clarify medication plans, and help connect you with additional services based on your needs.

Contact Vertivea Health to discuss your needs or refer someone preparing to leave a hospital or treatment program. We will review the request, discuss service availability, and explain the next steps. With the patient’s permission, discharge teams may coordinate with us before discharge.
